Likewise Floating Support Referral
Hello! We want to offer a wide range of ways that you can refer to Floating Support, to keep our service accessible. If one of the following options doesn’t work, please do get in touch with our team.
To make a referral, please either
- Fill out the referral form online – below
- Call the Likewise office on 0207 278 4437, and a member of our team will help you submit the form between Monday-Friday 9am-5pm.
- Pop into our hub at 8 Fairhazel Gardens, NW6 3SG, for some support from our team.
- Email across a pdf version of the form to contactus@likewise.org.uk or send a paper copy to our address. Pdf version is here Likewise_Support-Referral_Form
Please note before referring: we are, at this time, only able to work with referrals where there is already a clear funding plan in place at the time of referring – either in the form of an established care package or an ability to self-fund. For other clients who are looking for support but don’t have funding at the moment, we encourage you to make a referral to either Adult Social Care or a social worker placed in a mental health team. Please note that Likewise can’t provide support for clients to get funding.
